  目的:探讨分析四肢联动训练对SCI患者躯体功能及心理控制源的影响。方法:选择SCI损伤程度为D级的40例患者为研究对象,将其随机分为观察组和对照组各20例;其中对照组予以常规康复训练,观察组在对照组基础上予以四肢联动训练;比较2组治疗前后的脊髓损伤神经学分类国际标准(ISNCSCI)量表、脊髓损伤步行指数Ⅱ(WISCI Ⅱ)量表、脊髓独立性评定Ⅲ(SCIM Ⅲ)量表、表面肌电(sEMG)及多维度健康状况心理控制源量表(MHLC)五个参数的变化情况。结果:治疗前,观察组与对照组ISNCSCI量表中各指标、WISCI II评分、SCIM III评分、竖脊肌肌电数值、MHLC量表的各控型评分均无显著性差异。治疗2周后,2组下肢运动分数、WISCI II评分均较治疗前明显增加(P<0.05),且观察组治疗后WISCI II评分明显高于对照组(P<0.05),对照组轻触觉分数较治疗前明显增大(P<0.05),观察组轻触觉分数较治疗前无显著性差异;观察组SCIM III评分较治疗前及对照组明显提高(P<0.05);观察组机遇控型得分均较治疗前及对照组明显下降(P<0.05),对照组SCIM III评分治疗前后比较无显著性差异。2组上肢运动、针刺觉分数及左、右侧胸段竖脊肌、左侧腰段竖脊肌数值较治疗前无显著性差异,对照组右侧腰段竖脊肌数值较治疗前无显著性差异,仅观察组右侧腰段竖脊肌数值较治疗前明显增大(P<0.05)。结论:通过四肢联动训练可有效促进SCI患者康复,提高下肢运动功能、躯干功能及步行能力,改善日常生活活动能力及调整心理控制源倾向,可在临床上推广。
英文摘要:
  Objective: To explore and analyze the influence of limb linkage training on the physical function and locus of control in patients with spinal cord injury (SCI). Methods: A total of 40 patients with SCI of grade D were selected as the research objects, and randomly divided into two groups: the experimental group (n=20) and the control group (n=20). The control group and experimental group received routine rehabilitation training, and the experimental group was given limb linkage training additionally. The ISNCSCI scale, WISCI II scale, SCIM Ⅲ score, surface electromyography (sEMG) and MHLC were observed and compared between the two groups before and after treatment. Results: Before treatment, there were no significant differences between the experimental group and the control group in the ISNCSCI scale, WISCI II scale, SCIM Ⅲ score, erector spinae myoelectric value, and MHLC. At 2nd week after treatment, the lower limb movement scores in the experimental group and the control group were significantly different from those before treatment, the light touch score in the control group was significantly different from that before treatment, the WISCI II scores in the experimental group and the control group were significantly higher than those before the treatment, the WISCI II scores in the experimental group were significantly higher than those in the control group, the SCIM Ⅲ scores in the experimental group were significantly increased compared to those before treatment, the value of the right lumbar erector spinae in the experimental group was significantly different from that before treatment, and the chance control score in the experimental group was significantly lower than before treatment and significantly lower than in the control group (P<0.05). Conclusion: The linkage training of limbs can effectively promote the rehabilitation of patients with SCI, improve lower limb motor function, trunk function and walking ability, improve the activities of daily living and adjust the tendency of psy-chological control sources, which can be promoted in clinical practice.
