| 目的：采用加拿大作业表现量表（COPM）分析脑卒中后肢体功能障碍患者作业活动需求，实施以患者为中心的治疗模式，观察疗效；并调查患者和康复治疗师应用COPM的体会。方法：脑卒中患者51例，均在入院时应用COPM全面评测作业活动需求，以此制定康复目标和治疗计划，实施以患者为中心的作业治疗模式；比较治疗前后作业活动的表现和满意度；同时发放调查问卷，调查患者和治疗师应用COPM的体会。结果：患者自理活动需求明显多于生产和休闲活动需求(P<0.01)；作业活动表现评分和满意度评分均较治疗前明显提高(P<0.05)；调查表明COPM涉及的问题更加细致、全面和实际，能够确认患者亟待解决的作业活动问题。结论：COPM简单易行，能确认患者亟待解决的作业活动，对临床治疗有指导意义。 |

| Objective:To observe the effect of the application of Canadian occupational performance measure (COPM) on the rehabilitation therapy of stroke patients and analyze the feedback of patients and therapists.Methods:COPM was applied to evaluate occupational needs for 51 stroke patients in hospital,and rehabilitation aim and plan were made to carry out the patients-centered occupational therapy.The performance and degree of satisfaction of the occupational therapy were compared.The patients and therapists were investigated.Results:The problems of self-care activities were more than those of productive activities and leisure activities (P<0.01).The scores of performance and satisfaction of occupational therapy were significantly increased after rehabilitation therapy (P<0.05).Investigation showed the content of COPM was more careful,comprehensive and practical,and COPM could better solve the problems of occupational activities in stroke patients.Conclusion:COPM is easy to operate,helpful to solve the problems of occupational activities,which can guide the clinical practice. |
