| 目的：探讨康复期脊髓损伤(SCI)患者家属心理社会压力现状及患者相关影响因素。方法：应用家属压力量表对78例SCI患者家属压力进行评定，同时采用自制一般问卷、抑郁自评量表、焦虑自评量表、改良巴氏指数分别评估对应患者的一般情况、心理状态以及日常生活活动能力；再以SCI患者家属压力总分为因变量，以患者年龄、性别、病程、损伤程度、功能独立水平、心理状态等因素为自变量进行多元逐步回归分析。结果：SCI患者家属普遍面临较大压力，家属压力总分（36.1±11.4），家属压力集中体现在生活不悦方面，尤其以无暇顾及家务、假期无法分身以及社交活动受到影响频率最高。经多元逐步回归分析,最后进入回归方程具有显著性影响的变量为患者的抑郁症状（β=0.307,P=0.002）、病程(β=0.032,P=0.007)、损伤严重程度 (β=0.132,P=0.004)。结论：SCI患者家属普遍面临较大心理社会压力，尤其受病程、患者抑郁状态以及损伤严重程度影响。 |

| Objective:To investigate the psychosocial burden of family caregivers of the patients with spinal cord injury (SCI) in convalescent and to identify the patient-related factors.Methods:A 15-item relatives' stress scale (RSS) was applied to 78 family caregivers of SCI patients to measure the psychosocial burden.Meanwhile,the demographic information as well as the functional assessments on psychological symptoms,independence in activities of daily living of the patients was collected through the self-made questionnaire,Self-rating Anxiety Scale,Self-rating Depression Scale and Modified Barthel Index.The burden character was described with mean and standard deviation,and then a multiple stepwise regression was applied to identify the significant covariance.Results:Most of the families enrolled endured a moderate to severe burden with the mean sum score of RSS being 36.1±11.4,and the mostly affected aspect was life upset.The highly complained items included difficulties to get away on holiday,social life disrupted and upset in household activities.Through the multiple stepwise regression,the higher burden was significantly associated with the depression status (β=0.307,P=0.002),the onset duration (β=0.032,P=0.007) and injury severity (β=0.132,P=0.004).Conclusion:Family caregiver of patient with SCI commonly endured with moderate to considerable psychosocial burden and all of the onset duration,the depression symptom and the severity of injury associated with it. |
