| 目的：观察运用Mulligan手法治疗腰椎小关节紊乱的临床疗效。方法：82例腰椎小关节紊乱患者随机分为观察组和对照组各41例，观察组采用Mulligan手法治疗，对照组采用腰部斜扳法治疗。2组均隔日治疗1次，共治疗3次。治疗前后进行视觉模拟评分法（VAS）和日本骨科协会腰背痛疗效评价系统（JOA）评定，并进行肌酸激酶（CK）和乳酸脱氢酶（LDH）检验。结果：治疗3次后，2组VAS评分均较治疗前明显下降（P<0.05），且观察组更低于对照组（P<0.05）。治疗后，观察组JOA显效率及总有效率较对照组明显升高（P<0.05）。治疗后，2组血清CK及血清LDH值均较治疗前明显下降（P<0.05），且观察组更低于对照组（P<0.05）。结论：Mulligan手法对腰椎小关节紊乱的疗效明显，可以有效地减轻疼痛，调整腰椎关节，调节腰部周围肌群平衡，从而恢复腰部运动功能。 |

| Objective: To observe the clinical curative effects of using Mulligan technique to treat lumbar small joint disturbance. Methods: Eighty-two cases of lumbar small joint disorders were randomly divided into observation group (n=41) and control group (n=41). The observe group was given Mulligan technique treatment, and the control group was given the waist oblique pulling method treatment. Treatment was carried out once every other day for three times. Two groups were assessed with Visual Analog Scale (VAS) and JOA low back pain efficacy evaluation system before and after treatment. Serum creatine kinase (CK) and serum lactate dehydrogenase (LDH) were determined. Results: After 3-time treatment, VAS scores were significantly decreased in both two groups as compared with those before treatment (P<0.05), more significantly in the observation group than in the control group (P<0.05). After treatment, JOA excellent effective rate and total effective rate were significantly increased in the observation group as compared with those in the control group (P<0.05). After treatment, serum CK and LDH levels were significantly decreased as compared with those before treatment (P<0.05), more significantly in the observation group than in the control group (P<0.05). Conclusion: Mulligan technique can treat lumbar facet joint disorders effectively, alleviate the pain effectively, correct the lumbar vertebrae, and balance the muscle around the waist, thus recovering the kinetism of waist. |
