  目的:探讨应用动静态平衡仪评定踝关节稳定程度的信度与效度,为临床应用提供客观依据。方法:慢性踝关节不稳(CAI)患者32例(CAI组)和正常人群30例(正常组),对CAI患者的患侧踝关节和正常人群单侧踝关节进行3次Pro-kin动静态平衡仪的本体感觉预评估、多轴本体感觉评估和Cumberland踝关节不稳定评价问卷评定,并将32例3次慢性踝关节不稳患者的本体感觉预评估指标和多轴本体感觉评估指标的均值分别与Cumberland不稳定评价问卷评分进行相关性分析,以检验动静态平衡仪的效度。收集62例受试者的3次评估指标采用等级相关系数(ICC)检验Pro-kin动静态平衡仪的信度。对正常组与CAI组多轴本体感觉评估结果采用两独立样本t检验来检验Pro-kin动静态平衡仪的灵敏度。结果:Pro kin动静态平衡仪多轴本体感觉评估指标:平均轨迹差,平均负重力量差与Cumberland踝关节不稳定评价问卷评分呈中度负相关(r=-0.421,-0.419,P<0.05);本体感觉预评估指标:前后稳定指数、左右稳定指数、A2-A6稳定指数、A4-A8稳定指数、圆周稳定指数与Cumberland踝关节不稳定评价问卷评分相关性弱(r=-0.057,-0.003,-0.013,-0.129,-0.164,P<0.05)。Pro-kin动静态平衡仪的重复测量信度ICC为0.831~0.959,95%可信区间集中;测量者间信度为0.798~0.961,95%可信区间集中。正常组与CAI组多轴本体感觉评估指标比较,差异具有统计学意义(均P<0.05)。结论:Pro-kin动静态平衡仪多轴本体感觉评估检测指标平均轨迹差和平均负重力量差可用来评估踝关节稳定性且具有较高的灵敏度,并且Pro-kin动静态平衡仪测量踝关节稳定性具有较高的信度。

  Objective: To study the reliability and validity of the dynamic and static balance instrument used to assess ankle's stability degree so as to provide objective basis for the clinical practice. Methods: The enrolled included 32 patients with chronic ankle instability (CAI group) and 30 normal people (normal group). The affected ankle in the CAI group and the unilateral ankle in the normal group were assessed by Pro kin body feeling pre-assessment of dynamic and static balance, multi axis ontology sensory evaluation and David kotok ankle instability evaluation questionnaire three times. The average values of Pro-kin body feeling pre assessment of dynamic and static balance and average values of multi axis ontology sensory evaluation were separately compared with those of David kotok unstable evaluation questionnaire scores in order to test the validity of Pro-kin dynamic and static balance instrument. The ICC was used to analyze the data of 62 cases for testing the reliability of Pro kin dynamic and static balance instrument. The David kotok ankle instability evaluation questionnaire scores in normal group and CAI group were compared by t test to test the sensitivity of Pro-kin dynamic and static balance. Results: For Pro kin dynamic and static balance instrument, multi axis proprioception evaluation indicators including the average trajectory difference and the average load power difference had a moderate negative correlation with David kotok ankle instability evaluation questionnaire score (r=0.421, and 0.419, P<0.05). Proprioception pre assessment indicators including front and behind stability index, left and right stability index, A2-A6 stability index, A4-A8 stability index, and circular stability index had weak correlation with David kotok ankle instability evaluation questionnaire score (r=-0.057, -0.003, -0.013, -0.129, -0.164, P<0.05). Pro-kin repetitive measure reliability (ICC) of dynamic and static balance was 0.831-0.959, and 95% CI was concentrated. The reliability among the investigators was 0.798-0.961,and 95% CI was concentrated. The multi axis proprioception evaluation indexes showed no statistically significant difference between the normal group and CAI group (P<0.05). Conclusion: The average trajectory difference and average load difference for multiaxis proprioception assessment test in Pro-kin dynamic and static balance instrument can be used to assess the stability of ankle joint and have high sensitivity, and Pro-kin dynamic and static balance instrument has high reliability in measuring ankle stability.
