  目的:评价中等强度有氧运动对阿尔茨海默病(AD)患者认知功能、精神行为症状、生活质量的改善作用,为临床探索更有效的AD治疗手段提供参考。方法:AD患者48例,随机分为2组各24例,运动组给予每周3次,每次40min,持续3个月的中等强度有氧运动操训练。对照组仅要求受试者完成日常活动,如散步等,不接受有氧训练。治疗前、3个月训练结束时,随访3个月后分别对2组患者进行认知评估量表(MMSE、ADAS-cog、韦氏逻辑记忆量表、韦氏数字广度量表、数字连线A、数字连线B、数字符号量表)、神经精神量表(老年抑郁量表)、生活质量量表(阿尔茨海默病生活质量量表、社会功能量表、平衡功能量表)评估。结果:治疗3个月及随访3个月后,运动组患者MMSE评分、韦氏记忆广度和韦氏数字记忆评分及符号数字评分均较治疗前及对照组明显提高(P<0.05),ADAS-cog、数字连线A及数字连线B评分均较治疗前及对照组明显降低(P<0.05);对照组治疗3个月后仅MMSE评分较治疗前有所提高(P<0.05),随访3个月后,韦氏逻辑记忆量表评分较治疗前有显著降低(P<0.05)。治疗3个月后,2组老年抑郁量表评分治疗前后及组间比较均差异无统计学意义,随访3个月后,运动组老年抑郁量表评分较治疗前、治疗3个月时及对照组均明显下降(P<0.01);对照组老年抑郁量表评分治疗前后各时间点比较均差异无统计学意义。治疗3个月及随访3个月后,2组患者生活质量、社会功能及Berg平衡量表评分治疗前后及组间比较均差异无统计学意义。结论:表明有氧运动对AD患者的认知功能,尤其是记忆功能、精神行为症状有改善作用。

  Objective: To evaluate the effects of moderate intensity aerobic exercise on cognitive function, mental behavior symptoms and quality of life in patients with Alzheimer disease (AD), and to provide reference for effective AD treatment. Methods: Forty-eight participants were randomly divided into aerobic exercise group (n=24) and control group (n=24). Patients in exercise group were given 40-min moderate intensity aerobic exercise training, 3 times a week for 3 months. Cognition function scale (Minimum Mental State Examination, Alzheimer Disease Assessment Scale-cognitive section, Wechsler Memory Scale, Wechsler digit span scale, Digital connection A, Digital connection B, Digital symbol scale), neuropsychological scale (Geriatric depression scale) and quality of life scale (Quality of Life Alzheimer's Disease, Functional Activities Questionnaire, Berg balance scale ) were performed at baseline and after 3 and 6 months. Results: The patients were followed up and trained according to the requirements. No significant differences were found in demography characteristics and cognition scores between the exercise group and the control group at baseline (P>0.05). The exercise group scored significantly higher in MMSE, the Wechsler logical memory story and Wechsler digit span test, while lower in ADAS-cog, Digital connection A, Digital connection B than in control group after 3-month aerobic training and 6-month follow-up (P<0.05). No significant difference was observed in FAQ, Qol AD, Berg scores between exercise group and control group after 3-month aerobic training and 6-month follow-up (P>0.05). Conclusions: The aerobic exercise can improve the cognitive function, especially memory function and mental behavior symptoms in AD patients.
