  目的:探讨励-协夫曼运动治疗(LSVT-BIG)对原发性帕金森病患者步行功能和生活质量的影响。方法:原发性帕金森病患者40例,按随机数表法分为BIG疗法组和常规康复组各20例,分别给予8周的康复治疗,于治疗前和治疗8周后采用国际运动障碍协会帕金森病综合评价量表第三部分(MDS-UPDRS Ⅲ)、“起立-行走”计时测验(TUGT)、10m步行试验(10MWT)、帕金森病患者生活质量问卷(PDQ-39)对2组患者进行疗效评价。结果:治疗8周后,与治疗前比较, 2组MDS-UPDRS Ⅲ总分减少(P<0.05),TUGT时间减少(P<0.05),10-MWT时间缩短(P<0.05),步长增加(P<0.05),PDQ-39评分减少(P<0.05);组间比较,BIG组10MWT步行时间、步长、MDS-UPDRS Ⅲ总分及PDQ-39评分改善较对照组治疗前后改善更佳(P<0.05)。结论:LSVT-BIG疗法能有效改善帕金森病患者步行功能和生活质量。

  Objective: To explore the effect of Lee Silverman Voice Treatment (LSVT)-BIG on motor function, walking function and quality of life in patients with Parkinson’s disease. Methods: A total of 40 patients with primary Parkinson’s disease in Hoehn-Yahr stages 1-3 were selected and divided into the BIG therapy group and the conventional rehabilitation group according to the random number table method. Before treatment and after 8 weeks of treatment, the Unified Parkinson’s Disease Comprehensive Evaluation Scale, standing-walking time test (TUGT), 10-meter walking test (10MWT), 39 items of Parkinson’s disease questionnaire (PDQ-39) were used to evaluate the efficacy. Results: After 8 weeks of training, the total scores of MDS-UPDRS Ⅲ decreased (P<0.05), TUGT time shortened (P<0.05), 10-MWT time shortened (P<0.05), step length increased (P<0.05), and PDQ-39 score decreased as compared with those before treatment, indicating that conventional rehabilitation treatment and BIG therapy could improve walking function and quality of life in patients with Parkinson’s disease. The improvement of 10-MWT time, 10-MWT step length, PDQ-39 score and MDS-UPDRS Ⅲ total score in LSVT-BIG group was better than that in conventional treatment group, and the difference was statistically significant (P<0.01). Conclusions: LSVT-BIG therapy can effectively improve the walking function and quality of life of patients with Parkinson’s disease.
