  目的:研究经颅直流电刺激(tDCS)对脑卒中后注意障碍患者的注意功能影响及可能的机制。方法:选取2021年10月~2022年6月在徐州市中心医院康复科进行治疗的60例卒中后注意障碍患者,随机分为观察组与对照组各30例。2组患者均进行常规认知康复训练,观察组在此基础上予以tDCS刺激,每周5次,每次20min,共4周;对照组参照观察组,但仅在初始和最后30s内施加电流,余同观察组。采用数字广度测试(DST)和字母删除试验分别对患者注意广度和持久度进行评估,采用符号数字模式测试(SDMT)对患者注意转移能力进行评估,采用蒙特利尔认知评估量表(MoCA)及改良Barthel指数(MBI)分别对患者认知功能及日常生活能力进行评估。并评估2组患者事件相关电位P300潜伏期和波幅的改善情况。结果:治疗4周后,2组患者MoCA、MBI、SDMT、DST评分、字母正确删除数、P300和波幅均增加(均P<0.05),P300潜伏期缩短(P<0.05);观察组MoCA、MBI、SDMT、DST评分、字母正确删除数、P300波幅均高于对照组(均P<0.05),P300潜伏期短于对照组(P<0.05)。结论:在常规治疗基础上进行tDCS治疗可以改善患者注意功能,提高认知及日常生活能力水平。

  Objective: To study the effect of transcranial direct current stimulation (tDCS) on attention function in patients with cognitive impairment after stroke and its possible mechanism. Methods: A total of 60 patients with attention impairment after stroke in the Department of Rehabilitation Department, Xuzhou Central Hospital from October 2021 to June 2022 were randomly divided into an experimental group and a control group (n=30 each). Both groups were treated with routine cognitive rehabilitation training. Additionally, the experimental group was given tDCS 5 times a week, 20 min each time for 4 weeks, and the control group was given electric current at the initiation and last 30 s. The attention span and persistence of patients were evaluated by digit span test (DST) and Alphabet Deletion Test, and the ability of attention transfer was evaluated by symbolic number pattern test (SDMT). The cognitive function and activities of daily living were evaluated by Montreal Cognitive Assessment scale (MoCA) and modified Barthel Index (MBI). At the same time, the improvement of latency and amplitude of event-related potential P300 in two groups were evaluated. Results: After treatment, the scores of MoCA, MBI, SDMT, DST, correct deletion, P300 latency and amplitude in the two groups were better than those before treatment (P<0.05), and the scores of MoCA, MBI, SDMT, DST, correct deletion, P300 latency and amplitude in the experimental group were higher than those in the control group after treatment (all P<0.05). Conclusion: tDCS therapy on the basis of routine treatment can improve patients’ attention function and improve the level of cognition and activities of daily living.
