  目的:探讨基于前庭-眼反射的凝视稳定训练对提高老年人平衡功能的研究分析。方法:选取体检健康的100例老年人作为研究对象,按照随机数字表法分为干预组和对照组,每组50例。对照组进行常规物理治疗训练,干预组在对照组的基础上再进行凝视稳定训练。干预前、干预后、干预后3个月采用Berg平衡量表(BBS)来评定受试者的平衡能力、采用特定活动平衡信心量表(ABC)测定所有受试者的信心情况、采用平衡仪测定所有受试者的跌倒风险;干预前后进行站立-行走计时测试(TUG);并对所有受试者进行1年的随访,记录并计算跌倒率。结果:干预4周及干预后3个月,2组的BBS及ABC评分均呈逐渐升高趋势(P<0.05),且各时间点干预组的BBS及ABC评分显著高于对照组(P<0.05);干预4周及干预后3个月,2组的TUG时间及跌倒率均呈逐渐下降趋势(P<0.05),且各时间点干预组的TUG时间及跌倒率显著低于对照组(P<0.05);随访1年结果表明,干预组跌倒率显著低于对照组(6.00%、22.00%,P<0.05)。结论:基于前庭-眼反射的凝视稳定训练可以有效提高老年人的平衡功能和信心,降低其跌倒风险,提高生活质量。

  Objective: To explore the effect of gaze stabilization training based on vestibule-eye reflex on the balance function of the elderly. Methods: A total of 100 elderly people who were physically healthy in Yixing NO.2 People’s Hospital from December 2020 to January 2022 were selected as the subjects of this study. They were divided into intervention group and control group according to the method of random number table, with 50 cases in each group. The control group received general physical therapy training, and the intervention group received gaze stabilization training on the basis of the control group. Before, immediate and 3 months after the intervention, the Berg Balance Scale (BBS) was applied to assess the balance ability of the subjects, the specific activity balance confidence scale (ABC) was used to measure the confidence of all subjects, and the balance instrument was used to measure the fall risk of all subjects. Before intervention, immediate after intervention, and 3 months(after) of intervention, timed up and go test (TUG) was performed. All subjects were followed up for 1 year, and the fall rate was recorded and calculated. The BBS score, TUG time, ABC score, fall risk and fall rate in the two groups were compared before, immediate and 3 months after the intervention. Results: The BBS score and ABC score in the intervention group and the control group increased obviously after the intervention, while the TUG time and fall risk decreased obviously after the intervention (P<0.05), the BBS score and ABC score after 3 months of intervention were obviously higher than those before and immediate after intervention, and the TUG time and fall risk were obviously reduced as compared with those before and immediate after intervention (P<0.05). After intervention and 3 months after intervention, the BBS score and ABC score in the intervention group were obviously higher than those in the control group, and the TUG time and fall risk were obviously reduced as compared with those in the control group (P<0.05). The results of one-year follow-upshowed that the fall rate of patients in the intervention group (6.00%, 3/50) was obviously lower than that in the control group (22.00%, 11/50) (χ2=5.316, P=0.021). Conclusion: The gaze stabilization training based on vestibule-eye reflex can effectively improve the balance function and confidence of the elderly, reduce their fall risk and improve their quality of life.
