| 目的：观察Mulligan 动态关节松动术联合超短波、针灸治疗膝骨关节炎的临床疗效。方法：将在我科就诊的膝骨关节炎患者60例随机分为观察组和对照组各30例，对照组给与超短波及针灸治疗，观察组进行超短波、针灸及Mulligan 动态关节松动术治疗，连续治疗8周，观察2组患者治疗前后WOMAC评分、平衡功能、关节间隙角、关节腔积液及滑膜厚度的变化。结果：治疗后2组患者WOMAC评分较治疗前下降，滑膜厚度变小，关节腔积液及关节间隙角减小，平衡功能包括稳定极限、单腿负重较前提高，左右摆幅、前后摆幅较前下降，差异均有统计学意义(P＜0.05)，且观察组较对照组各项指标改善明显，差异有统计学意义(P＜0.05)。结论：Mulligan动态关节松动术联合超短波及针灸治疗膝骨关节炎可明显改善关节功能、平衡功能及下肢力线，减少关节腔积液及滑膜厚度，疗效优于超短波及针灸治疗，值得临床推广。 |

| Objective: To observe the clinical effects of Mulligan’s mobilization-with-movement combined with ultrashort wave therapy and acupuncture in patients with knee osteoarthritis. Methods: Patients with knee osteoarthritis diagnosed and treated in our department were randomly divided into control group and observation group. The control group was given ultrashort wave therapy in combination with acupuncture treatment for 8 weeks, and the observation group was given Mulligan’s mobilization-with-movement based on the control group. Changes of WOMAC score, balance function, joint gap angle, thickness of the joint effusion and synovial were compared between the two groups. Results: The WOMAC score, the swing amplitude of left and right, front and back, the joint effusion and synovium thickness after treatment in both groups were lower than those before treatment. The joint gap angle decreased after treatment. The stability limit and the weight bearing on one leg were significantly increased after treatment in both groups (P<0.05), and those in observation group was significantly greater than in the control group (P<0.05). Conclusion: The Mulligan’s mobilization-with-movement combined with ultrashort wave therapy and acupuncture can significantly improve joint function, balance and lower limb alignment, reduce knee joint pathological changes in patients with knee osteoarthritis, and the curative effect is better than ultrashort wave and acupuncture treatment alone. It is expected to be promoted for knee osteoarthritis. |
