  目的:观察重复经颅磁刺激(rTMS)结合反复促通疗法对脑卒中后下肢运动功能的影响。方法:采用随机数字表法将80例符合纳入标准的脑卒中患者分为对照组40例和观察组40例。2组均接受常规康复与rTMS治疗,观察组在此基础上再给予反复促通疗法;每周6次,持续4周。治疗前后分别采用Fugl-Meyer量表下肢部分(FMA-LE)、功能性步行量表(FAC)、Berg平衡量表(BBS)及10m最大步行速度测试(10MWT)评定患者下肢运动功能。结果:治疗4周后,2组患者FMA-LE、FAC、BBS评分及10MWT均较治疗前有所提高(P<0.05),且观察组各项评分及10MWT均高于对照组(P<0.05)。结论:重复经颅磁刺激结合反复促通疗法治疗脑卒中患者可有效改善下肢平衡能力及运动功能,促进步行功能恢复。
英文摘要:
  Objective: To observe the effect of repetitive transcranial magnetic stimulation (rTMS) combined with repetitive facilitative exercise on lower limb motor function after stroke. Methods: A random number table was used to divide 80 stroke patients who met the inclusion criteria into a control group of 40 cases and an observation group of 40 cases. Both groups received routine rehabilitation and rTMS treatment, while the observation group received repetitive facilitative exercise on this basis, 6 times a week for 4 weeks. Before and after treatment, the lower limb motor function of patients was evaluated using the Fugl Meyer Scale (FMA-LE), Functional Walking Scale (FAC), Berg Balance Scale (BBS), and 10m Maximum Walking Speed Test (10MWT). Results: After 4 weeks of treatment, the scores of FMA-LE, FAC, BBS, and 10MWT in both groups were improved compared to those before treatment (P<0.05), and those in the observation group were higher than those in the control group (P<0.05). Conclusion: The combination of rTMS and repetitive facilitative exercise can effectively improve lower limb balance and motor function, and promote the recovery of walking function in stroke patients.
