 脑卒中后偏瘫步态的康复治疗主要分为常规康复治疗和神经调控治疗,其中经颅磁治疗因作用时间短、强度低等优点逐渐成为研究热点,但目前对于促进下肢运动功能恢复的刺激靶点选择仍较单一。本文基于小脑-大脑神经环路及其在脑卒中后偏瘫患者中的研究进行综述,总结及探究促进下肢运动功能恢复可能的新思路或新靶点。经颅磁治疗可通过调节双侧大脑半球间兴奋性的对称性或患侧脑区的兴奋性而影响对侧小脑,也可通过刺激小脑而影响小脑-大脑神经环路进而改善脑卒中患者下肢运动功能。同时干预下肢运动皮质区和小脑进行双靶点刺激,或将为卒中后偏瘫患者下肢运动功能的恢复提供新思路。 |

Rehabilitation treatment of hemiplegic gait after stroke is mainly divided into conventional rehabilitation treatment and neuromodulation treatment,in which transcranial magnetic therapy has gradually become a research hotspot due to the advantages of short action time and low intensity, etc. However, the current choice of stimulation targets for promoting the recovery of lower limb motor function is still relatively single. Based on the cerebellar-cerebral neural circuit, this paper summarizes and explores the possible new ideas or new targets to promote the recovery of lower limb motor function. We reviewed and analyzed the relevant literature by searching the key words including stroke, lower limb motor function, hemiplegic gait, cerebellar-cerebral neural circuit, transcranial magnetic stimulation, and rehabilitation. Transcranial magnetic therapy can affect the contralateral cerebellum by adjusting the symmetry of excitability between the bilateral cerebral hemispheres or the excitability of the brain region on the affected side, and it can also affect the cerebellar-cerebellar neural circuit by stimulating the cerebellum to improve the lower limb motor function of stroke patients. Simultaneous dual-target stimulation of the lower limb motor cortex and cerebellum may provide new ideas for the recovery of lower limb motor function in hemiplegic patients after stroke. |
