  目的:观察反重力跑台训练对脑卒中偏瘫患者的平衡功能、步态及日常生活能力的影响。方法:将38例脑卒中偏瘫患者随机分为对照组和观察组各19例。2组均予常规康复训练,对照组加用常规步行训练,观察组加用反重力跑台步行训练。在治疗前后,采用动静态平衡分析训练系统及Berg平衡量表(BBS)进行平衡功能评定,采用步态评估系统进行步态评定,采用Barthel指数(BI)进行日常生活能力评定。结果:治疗8周后,2组MedTrack动静态平衡分析的X轴重心偏移、Y轴重心偏移、暴露面积、动摇角度均较治疗前明显降低(P<0.05),且观察组低于对照组(P<0.05);2组的BBS评分均较治疗前明显提高(P<0.05),且观察组高于对照组(P<0.05);治疗后,2组的患侧单支撑相百分比、步速、步幅均较治疗前明显提高(P<0.05),足偏角均较治疗前明显降低(P<0.05),观察组的患侧摆动相百分比较治疗前明显提高(P<0.05),且治疗后观察组的患侧单支撑相百分比、摆动相百分比、步速、步幅均高于对照组(P<0.05),足偏角低于对照组(P<0.05);治疗后,2组的BI评分均明显高于治疗前(P<0.01),且观察组高于对照组(P<0.05)。结论:反重力跑台训练可有效改善脑卒中偏瘫患者的平衡功能、步态及日常生活能力,对脑卒中偏瘫患者的康复治疗有积极作用。

  Objective: To observe the effect of anti-gravity treadmill training on balance function, gait and daily life ability in hemiplegic patients with stroke. Methods: Totally, 38 hemiplegic patients with stroke were randomly divided into a control group and an experimental group with 19 cases in each group. Both groups received routine rehabilitation training, the control group received routine walking training, and the experimental group received anti-gravity treadmill walking training. Before and after treatment, balance function was assessed using the dynamic and static balance analysis training system and Berg Balance Scale (BBS), gait evaluation system was used to assess gait, and Barthel Index (BI) was used to assess daily life ability. Results: After treatment, the X-axis center of gravity offset, Y-axis center of gravity offset, exposure area and shaking angle in both groups were significantly reduced as compared with those before treatment (P<0.05), and those in the experimental group were lower than in the control group (P<0.05); the BBS scores in both groups were significantly higher than those before treatment (P<0.05), and those in the experimental group were higher than in the control group (P<0.05); After treatment, the percentage of single supporting phase, walking speed and stride length on the affected side in both groups were significantly improved as compared with those before treatment (P<0.05), the foot tilt angle was significantly lower than that before treatment (P<0.05), the percentage of swing phase on the affected side in the experimental group was significantly higher than that before treatment (P<0.05); After treatment, the percentage of single supporting phase, percentage of swing phase, walking speed and stride length in the experimental group were higher than those in the control group (P<0.05), and the foot tilt angle was lower than that in the control group (P<0.05); After treatment, the BI scores in both groups were significantly higher than those before treatment (P<0.01), and those in the experimental group were higher than in the control group (P<0.05). Conclusion: Anti-gravity treadmill train-ing can effectively improve the balance function, gait and daily life ability of hemiplegic patients with stroke, and has a positive effect on the rehabilitation treatment of hemiplegic patients with stroke.
