|  目的:探讨电针八髎穴与重复经颅磁刺激疗法相结合,在治疗脊髓损伤后神经源性膀胱功能障碍患者中对尿动力学及排尿功能的作用。方法:将80例脊髓损伤后神经源性膀胱功能障碍患者随机分为2组各40例,对照组采用重复经颅磁刺激治疗,观察组采用电针八髎穴联合重复经颅磁刺激治疗,治疗前及治疗8周后对2组患者的排尿功能、膀胱功能、尿流动力学进行评价,治疗8周后评价临床疗效及不良反应发生率。结果:治疗4、8、12周后,2组患者的每日排尿频次数、尿失禁次数、残余尿量、神经源性膀胱症状旁评分(NBSS)及国际下尿路症状评分(LUTS)评分均较治疗前呈显著下降趋势(P<0.05),且观察组患者各时间点评分明显低于对照组(P<0.05);治疗4、8、12周后,2组患者每日平均单次排尿量、膀胱最大容量及最大尿流率均呈显著上升趋势(P<0.05),且观察组患者各时间点评分明显高于对照组(P<0.05);治疗后,观察组患者的临床疗效显著优于对照组(P<0.05);治疗后2组患者的不良反应发生率比较差异无统计学意义。结论:电针八髎穴位与重复经颅磁刺激疗法相结合,在治疗脊髓损伤后神经源性膀胱功能障碍方面展现出显著的疗效,能够有效改善患者的尿流动力学参数及排尿功能,还具备较高的安全性。 |

| Objective: To investigate the effect of Baliao electroacupuncture combined with repeated transcranial magnetic stimulation on urodynamics and urination function in patients with neurogenic bladder dysfunction after spinal cord injury. Methods: A total of 80 patients with neurogenic bladder dysfunction after spinal cord injury were randomly divided into two groups, with 40 cases in each group. The control group was treated with repetitive transcranial magnetic stimulation, while the observation group was treated with electroacupuncture at Baliao points combined with repetitive transcranial magnetic stimulation. After 8 weeks of simultaneous treatment in both groups, the patients’ micturition function, bladder function, urodynamics, clinical efficacy, and incidence of adverse reactions were analyzed. Results: At 4, 8, and 12 weeks after treatment, the daily frequency of micturition, frequency of urinary incontinence, residual urine volume, Neurogenic Bladder Symptom Score (NBSS), and International Lower Urinary Tract Symptoms Score (LUTS) in both groups showed a significant decreasing trend compared to before treatment (P<0.05), and the scores at each time point in the observation group were significantly lower than those in the control group (P<0.05). At 4, 8, and 12 weeks after treatment, the average single micturition volume per day, maximum bladder capacity, and maximum urine flow rate in both groups showed a significant increasing trend (P<0.05), and the scores at each time point in the observation group were significantly higher than those in the control group (P<0.05). After treatment, the clinical efficacy in the observation group was significantly better than that in the control group (P<0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups after treatment. Conclusion: The combination of Balio acupuncture point and repeated transcranial magnetic stimulation therapy has shown significant efficacy in the treatment of neurogenic bladder dysfunction after spinal cord injury, which can effectively improve the urine flow parameters and urination function of patients, and also has a high safety. |
