  目的:探讨影响脑卒中患者偏瘫上肢日常使用量的相关因素。方法:纳入2023年1月至2023年10月我院收治的脑卒中患者170例进行回顾性分析,记录患者入院时的年龄、性别、病程、偏瘫侧别、偏瘫上肢运动功能(FMA-UE)、偏瘫上肢活动功能[偏瘫上肢功能测试香港版(FTHUE-HK)]、偏瘫上肢使用数量(MAL-AOU)、偏瘫上肢使用动作质量(MAL-QOM)、日常生活自理能力(MBI),卒中影响量表手功能子量表(SIS-Hand)及卒中影响量表ADL子量表(SIS-ADLs)的评定结果。以偏瘫上肢使用数量(MAL-AOU)为因变量,先进行单因素分析,再进行线性逐步回归分析,筛选影响脑卒中患者偏瘫上肢日常使用量的相关因素。结果:单因素分析显示:年龄、性别、病程、FMA-UE、FTHUE-HK、MAL-QOM、MBI、SIS-Hand、SIS-ADLs均与脑卒中患者偏瘫上肢的日常使用量相关(P<0.05);线性逐步回归分析显示MAL-QOM与脑卒中患者的偏瘫上肢日常量使用相关(P<0.05)。结论:年龄、性别、病程、FMA-UE、FTHUE-HK、MAL-QOM、MBI、SIS-Hand、SIS-ADLs是脑卒中患者偏瘫上肢日常使用量的相关因素。MAL-QOM是影响脑卒中患者偏瘫上肢日常使用量的独立相关因素。

  Objective: To explore the factors related to amount of daily use of the hemiplegic upper limb after stroke. Methods: The clinical data of 170 stroke patients from January 2023 to October 2023 were retrospectively analyzed, including age, gender, course of disease, paralyzed side, hemiplegic upper limb motor function (Fugl-Meyer Assessment-Upper Extremity, FMA-UE), hemiplegic upper limb activity function (Functional Test for the Hemiplegic Upper Extremity-Hong Kong, FTHUE-HK), Motor Activity Log-Amount of Use (MAL-AOU) and Motor Activity Log-Quality of Movement (MAL-QOM), Modified Barthel Index (MBI), Stroke Impact Scale-ADLs (SIS-ADLs) and Stroke Impact Scale-Hand (SIS-Hand). The MAL-AOU was used as the dependent variable, the relevant influencing factors of daily use of hemiplegic upper limb in stroke patients were filtered out by univariate analysis and linear stepwise regression analysis. Results: The result of univariate analysis showed that age, gender, course of disease, FMA-UE, FTHUE-HK, MAL-QOM, MBI, SIS-ADLs and SIS-Hand were correlated with the daily use of hemiplegic upper limb in stroke patients (P<0.05). Linear stepwise regression analysis showed that MAL-QOM was associated with the amount of daily use of the hemiplegic upper limb in stroke patients (P<0.05). Conclusion: Age, gender, course of disease, FMA-UE, FTHUE-HK, MAL-QOM, MBI, SIS-Hand and SIS-ADLs are factors associated with the daily use of hemiplegic upper limbs in stroke patients. Among these variables, MAL-QOM has been identified as an independent factor influencing the daily use of hemiplegic upper limbs in this population.
