  目的:通过观察针刺百会、四神聪干预MCAO模型大鼠脑组织的病理及脑缺血半暗带组织中IL 1、IL 6、TNF α表达水平的检测,探讨针刺百会、四神聪穴对MCAO模型大鼠的抗炎作用。方法:SPF级SD大鼠60只,采用分层随机法分为假手术组(15只)、造模组(45只)。模型复制成功后,将造模组大鼠随机分为非穴区对照组和针刺组。针刺组给予百会、四神聪穴针刺干预;非穴区对照组于穴区周边5mm处进行针刺皮肤干预。末次针刺后1h,评定神经功能缺损评分;处死大鼠,取脑组织,显微镜下观察其病理改变,并检测缺血半暗带脑组织IL 1、IL 6、TNF α含量检测。结果:干预后,3组神经功能缺损评分比较,非穴区对照组高于针刺组(P<0.01),针刺组高于假手术组(P<0.01)。假手术组脑组织病理切片未见异常,针刺组梗死灶局限,梗死灶周围炎性细胞数量较非穴区对照组明显减少。缺血半暗带组织中IL 1、IL 6、TNF α含量:与假手术组比较,非穴区对照组和针刺组显著升高(P<0.01),与非穴区对照组比较,针刺组显著降低(P<0.01)。结论:针刺百会、四神聪穴可明显降低MCAO模型大鼠神经功能缺损评分,减小梗死百分比,减轻炎症浸润程度,降低半暗带脑组织中细胞因子含量,具有一定的抗炎作用。

  Objective: To explore the anti inflammatory effect of acupuncture at Baihui and Sishengcong on MCAO model rats by observing the pathological changes of brain tissue and the expression level of IL 1, IL 6 and TNF α in penumbra tissues in MCAO rats. Methods: Sixty SPF SD rats were randomly divided into sham operation group (n= 15) and model group (n=45) by the stratified random sampling method. After successful replication of the model, model group rats were randomly divided into non acupoint control subgroup and acupuncture subgroup. The acupuncture subgroup was given the acupuncture at Baihui and SishenConong, and the non acupoint control subgroup was treated with acupuncture skin intervention at 5 mm around the acupoint area. At 1 h after the last acupuncture, the scores of nerve function defect were evaluated; the rats were killed, the pathological changes were observed under a microscope, and the contents of IL 1, IL 6 and TNF α in ischemic penumbra tissues were measured. Results: After intervention, the scores of nerve function defect in non acupoint control subgroup were significantly higher than in acupuncture subgroup (P<0.01), and those in acupuncture subgroup were significantly higher than in sham operation group (P<0.01). The pathological sections of the brain tissue in the sham operation group had no abnormal changes, the infarction focus was localized in the acupuncture subgroup, and the number of inflammatory cells around the infarction focus was significantly less in acupuncture subgroup than in non acupoint control subgroup. The contents of IL 1, IL 6 and TNF α in ischemic penumbra tissues of non acupoint control subgroup and acupuncture subgroup were significantly increased as compared with those in sham operation group (all P<0.01), and those in acupuncture subgroup were significantly decreased as compared with those in non acupoint control sbugroup (P<0.01). Conclusion: Acupuncture at Baihui and SiShenCong can significantly reduce the neurological deficit score, the percentage of infarction, the degree of inflammatory infiltration and the contents of cytokines in the penumbra brain tissues of MCAO model rats, which has certain anti inflammatory effect.
